What is Diabetes Care?
Across the Eastern Metropolitan Region approximately 2-5% of people have either Type 1 or Type 2 diabetes. Diabetes remains the number one cause of avoidable hospitalisations across the catchment. In the 2014-15 financial year, there were 18,290 avoidable diabetes related hospital admissions, taking up 123,261 hospital bed days.
What is OEPCP doing?
Diabetes is a multi-faceted condition that can require different types of healthcare professionals and systems to co-ordinate care appropriately in managing an individual’s diabetes. We are committed to working towards a system that has a coordinated approach to achieve improved health outcomes for patients.
