We are one of 28 Primary Care Partnerships in Victoria. Our partners include organisations working to improve the health and well-being of communities within Knox, Maroondah and Yarra Ranges. Our catchment is home to approximately 417,000 people.

The vision of the Outer East Primary Care Partnership is to “build an innovative, inclusive and reflective partnership that plans, collaborates and advocates with the community to improve health and well-being and reduce inequities”.
